OPUS Forms & Training

OHCS designed and maintains the OPUS data collection system. This system meets the requirements of various federal and state agencies by serving as a collaborative harbor of client-level needs and characteristics. By assisting our service agencies through data entry support, we are able to document client-level needs, identify client characteristics of those households receiving assistance, and assist low-income individuals and households in efforts to seek additional and alternative resources. This data is at agency, community, and state levels.

OPUS meets OHCS data collection and business processing needs. The system is available to our service partners free of cost.

Partner Resources

This fillable form is to be signed by a local service agency representative who has been trained on the System Administrator roles of the OPUS system. A copy of the signed agreement is to be kept on file at the local agency while the original should be mailed to OHCS.
 
This document should be posted in all service agencies where data is collected using OPUS. This posting explains to clients how and why their information is collected and used.
 
This is the full version of what is outlined on the Privacy Posting. This document should be available to give to any client who requests a copy at the local service agency level.
 
This fillable form is to be signed by all users of the OPUS system and should be kept on file at the local agency.
 
This form is to be completed when an service agency, not previously in OPUS, requires access to the OPUS Database.​
