License Verification:  971-673-2700  |  info@omb.oregon.gov
Call or email for information on a Board licensee, including license number, date first licensed in Oregon, expiration date, license status (active, inactive, etc.), a licensee's disciplinary standing, mailing address, business address, business phone number, specialty, malpractice, and location of patient records. Limited to three names each day. 

License verifications are also available online. There is no cost and no limit for online verifications.

Call with questions about if or when you should file a complaint regarding a Board licensee. The Complaint Resource Staff will help you understand the Medical Practice Act and explain how to file a complaint. If the Board cannot address your needs, the Complaint Resource Staff may be able to refer you to the appropriate agency or organization.

Our Location


We are located in downtown Portland in the Crown Plaza Building on the corner of SW 1st Avenue and Clay Street. A ramp to the front entrance of the building is on the south side the stairs on SW 1st Avenue. The Board office is on the sixth floor.

The Crown Plaza parking garage is located directly across SW 1st Avenue with entrances to the parking structure on Clay and Market Streets. There is metered parking available on Clay Street. The location is also easily accessible by TriMet with streetcar, Max, and bus line stops nearby.
 
From I-5 (Northbound)
  • Take I-405 North ramp towards City Center/Beaverton (US-26W)
  • Take the Front Ave/SW Naito Parkway exit, exit number 1A
  • Merge onto SW Harbor Dr.
  • SW Harbor Dr will turn into SW Clay St/US-26W
  • Go through the light at Naito Parkway, the Crown Plaza building is one block further on the left. The parking garage is on Clay St. between 1st and Second Ave. 
     

From I-5 (Southbound)

  • Take exit number 300B, towards Oregon City (US-26E/OMSI (OR-99E)
  • Keep Right at the fork in the ramp, going over the Morrison Bridge
  • Take the ramp towards Front Ave (Naito Parkway)/Salem (I-5 South)
  • Keep Right at the fork in the ramp.
  • Merge onto SW Front Ave/SW Naito Parkway.
  • Turn Right onto SW Clay St/US-26W
  • The Crown Plaza building is one block further on the left. The parking garage is on Clay St. between 1st and 2nd Ave. 
     

From US-26E/Sunset Hwy (Eastbound)

  • Take the US-26 East/Market St. exit towards City Center
  • Stay straight to go onto SW Market St./US-26E
  • Turn LEFT onto SW Naito Parkway
  • Turn LEFT onto SW Clay St/US-26W
  • The Crown Plaza building is one block further on the left. The parking garage is on Clay St. between 1st and 2nd Ave. 
     

From I-84 (Westbound)

  • Take I-5 South ramp towards Salem
  • Keep Right at the fork in the ramp, going over the Morrison Bridge
  • Take the ramp towards Front Ave (Naito Parkway)/Salem (I-5 South)
  • Keep Right at the fork in the ramp. Merge onto SW Front Ave/SW Naito Parkway.
  • Turn Right onto SW Clay St/US-26W
  • The Crown Plaza building is one block further on the left. The parking garage is on Clay St. between 1st and 2nd Ave.
