Area Plans address the needs of older adults, adults with disabilities and their caregivers while assessing the strengths and weaknesses of current community resources available to them.
Area Agencies on Aging (AAAs) are required to complete Area Plans every four years. The plans are developed with public input and approved by the Oregon Department of Human Services.

Oregon Community Health Needs Assessments and Improvement Plans
Oregon's Office of Rural Health in partnership with the Oregon Health Authority's Transformation Center and Public Health Division, created an interactive map of the most recent Community Health Needs Assessment and Community Health Improvement Plans for Oregon's non-profit hospitals, local public health authorities and CCOs.
The National Council on Aging has developed visualization tools to help understand and support underserve d communities in each state. These visualization tools were created to help benefits counselors identify underserved communities of individuals in their geographic area who may be eligible for public benefits but are not enrolled in those programs. These tools may be helpful in making decisions about where to target outreach efforts,and creating culturally sensitive and linguistically appropriate outreach and application assistance.
