Navigate to
https://oregon.pmpaware.net and select “Create an Account”. Follow the directions as prompted to compete registration.
If you already have a PDMP account, please email PDMP staff at
pdmp.health@dhsoha.state.or.us to transition your account.
California PDMP Access Acknowledgement
California PDMP data will be searchable through the OR PDMP web portal starting mid-2024. California requires that all OR users read, sign, and retain for their own records, this acknowledgement. Users do not have to provide a copy of this acknowledgement to the OR or CA PDMPs.
Medical, Dental, and Pharmacy Director PDMP Access
As of January 2018, medical and pharmacy directors (dental directors as of January 2020) will be allowed access to the PDMP for overseeing the operations of their respective entities.
When registering, medical, dental and pharmacy directors must select the “Chief Medical Officer” role type.

Prescription Data submissions (uploaders) will be accepted through the Appriss Clearinghouse at
https://pmpclearinghouse.net.
If you had a query account on the OR PDMP system, please check your email for information regarding access to the new website. If you are unable to locate the email, check your mailbox’s junk folder. If you need to create an account, please navigate to
https://oregon.pmpaware.net and click Create an Account.
