Senate Bill 710 Reports

Senate Bill 710 was passed in 2021 and changed what is prohibited and permissible relating to restraint and involuntary seclusion of children receiving services from ODDS children's settings. The bill also added new reporting requirements for Host Homes and Children's Residential programs.

Incident Management Team (IMT)

Incident Management and Incident Management Teams as outlined in Oregon Administrative Rule 411-415-0055 are required for all Case Management Entities (CMEs). Incident Management focuses on identifying, reporting, triaging, investigating, resolving, tracking and trending serious incidents.

Statewide IMT quarterly reports and due dates

CMEs are required to submit a quarterly Incident Management report with their CME-specific data and summaries to ODDS using the Department-approved form above. Due dates and reports are listed below.
