Instructions for completing ODVA Forms
- Save the Adobe PDF form(s) to your computer or server. You can save typed data into the form. The PDF form contains interactive form fields. Use the Highlight Fields button to color code the form fields.
- Electronically sign and encrypt the document using a digital ID. Contact your county IT systems manager if you have any questions about Digital Signatures.
- Submit the completed form(s) as an email attachment(s) to Mae Wasson at mae.wasson@state.or.us.
