The Oregon Health Authority (OHA) and the Northwest Health Foundation (NWHF) have convened a diverse group of clinicians (primary care, mental health), patients, public health, and healthcare delivery technical experts from across Oregon to provide recommendations that will inform an approach to support broad implementation of Patient-Centered Primary Care Homes (PCPCH) in our state. The Taskforce will develop recommendations and action steps for infrastructure and technical assistance needed for broad-scale implementation of the PCPCH model throughout Oregon. Current Members of the Taskforce are listed below.
The work of the Taskforce includes:
- Review model options and expert recommendations about possible infrastructure approaches and technical assistance that could address the needs of Oregon in transforming primary care and healthcare delivery to achieve the triple aim of a healthy population, extraordinary patient care for everyone, and reasonable costs shared equally.
- Make recommendations for infrastructure and other solutions and resources that can help ensure successful transformation
- Collaborate with Oregon Health Authority and other stakeholders (i.e. statewide associations, communities, etc.) to identify cost-effective, efficient and innovative methods to support primary care in successful transition to new model of care delivery.
The first meeting of the Patient-Centered Primary Care Implementation Taskforce was held at Northwest Health Foundation on Thursday, July 14, 2011. OHA and NWHF encourage you to stay informed as this work begins. We invite ongoing input from the communities we serve.
