Become recognized

Many health clinics in Oregon already practice patient-centered primary care, and now they can be recognized for their commitment to better health, better care and lower costs.

1. Standards: Review the criteria measures to determine which standards your practice meets.

2. Self-assessment tool: This tool helps practices gather the information for the application.

3. Application: Complete and submit the application electronically.

4. Enhanced Payment: Oregon is working toward a system that rewards high quality, efficient care that results in better health outcomes. More information on this opportunity will be available in early 2012.
