(Use to upgrade to a regular SLP license after completing a clinical fellowship)

 For prospective SLPAs who need to complete their SLPA clinical fieldwork outside of an academic practicum.

Rules regarding licensure and supervision requirement of provisional speech-language pathology assistants are found in OAR Chapter 335 Division 85 & Division 95

 

Upgrade to Regular Speech-Language Pathology Assistant Application

For Provisional SLPAs who have completed their SLPA clinical fieldwork outside of an academic program and who are ready to upgrade to a full SLPA certificate.
