Office of Degree Authorization (ODA) Forms and Procedures, In-State 

New Applications for In-State Institutions 

Oregon statute requires degree-granting institutions to obtain ODA approval prior to offering degree programs in Oregon or to students in Oregon from outside the state. 

For In-State institutions located in Oregon applying for approval to offer degrees at a campus in Oregon the following steps are required:

For degree programs with multiple concentrations, a side-by-side comparison chart of all concentrations offered is required and must demonstrate the similarities and differences in courses and other graduation requirements for all concentrations.

Complete your application by including, in a separate folder, all of the supporting documents requested within the application forms:

  • Faculty and senior staff qualifications form.
  • Biographical information for members of the board of directors.
  • Facilities maps
  • Insurance and Financial documents
  • Excerpts from student and faculty policies and handbooks
  • Copy of the most current catalog or online links to the most current version of these resources

ODA Staff will contact you if further information is required to process your application.

Incomplete applications will not be processed. No application will be reviewed prior to payment of the invoiced fees. Please note that a separate specific program application form must be submitted for each degree and certificate program that is offered. 

For schools offering degree programs with multiple majors or concentration areas (these may also be called specializations or focus areas), a separate program application will be required for each of these individually identified areas of study and applications will not be processed if all concentrations offered by the school (whether online or on-campus) are not submitted for initial review. 

ODA will determine the extent of review required for subsequent re-authorization of existing programs based on current policy. 

Upon receipt of your application, ODA will send an invoice for the appropriate fees for review of each specific program application and authorization for a two-year period. Please allow six months for review and processing of your authorization to offer degrees’ programs in the State of Oregon. A site visit will be required prior to approval, or in the case of a new school, within the first three months of operation. 

Renewals for In-State Institutions

The application forms are the same for new or renewal applications for in-state or out-of-state institutions.

Surety Bonds and Letters of Credit

Oregon Statute requires degree-granting institutions oper​ating in or from Oregon to submit a Surety Bond or Letter of Credit (LOC). This requirement applies to institutions regulated by the Office of Degree Authorization. See the procedures below.
