Choice Model

Supporting Community Transitions for Individuals in Mental Health Residential Treatment

The Choice Model will promote the availability and quality of individualized community-based services and supports for adults with mental illness who:

  • Are in a mental health institution listed in Oregon Revised Statute 179.321, or
  • Are in a licensed community-based setting listed in ORS 443.400, or
  • Are in civil commitment under ORS 426, or
  • Were in civil commitment within the past 12 calendar months, and
  • Need treatment and community supports to prevent needing future residential treatment upon release.

This work will:

  • Provide services in the most independent environment possible and 
  • Minimize the use of long-term institutional care.

Please use these resources to ensure that adults in mental health residential treatment maintain their “home” CCO. (The "home" CCO is the CCO the individual had prior to placement in temporary residential treatment.)

​Funding categories for Choice Model services, when not covered by health insurance, include: 

  • ENCC / Transition Management / Mobile Crisis and other Clinical Staff costs (salary + benefits) 
  • Promote peer run and peer delivered services that is community based (development costs, salary + benefits, contract costs) 
  • Housing supports and rental assistance (eg: temporary housing, security deposit, set-up costs, furniture, etc.) 
  • Supported Employment / Vocational Rehabilitation
  • Assertive Community Treatment
  • Habilitative / Rehabilitative Mental Health Treatment Services
  • Medications
  • Barriers to Community Living / Engagement Supports for Recovery-Oriented Services (gym membership, bike, art supplies, personal items, work clothes; transportation not covered by health insurance, etc.)
  • Guardianship Cost

To unlicensed, non-congregate housing: 

  • ​6 points within 30 days of RTT
  • 3 points after 30 days of RTT

To licensed residential setting or unlicensed congregate housing: 

  • ​​3 points within 30 days of RTT
  • 1 point after 30 days of RTT

To transitional housing: 1 point

​From Secure Residential Treatment Facilities:

  • To unlicensed, independent living: 3 points
  • To non-secure, licensed residential setting: 1 point
  • To transitional housing: 1 point

From Residential Treatment Facility, Residential Treatment Home or Adult Foster Home:

  • To unlicensed, independent living: 3 points
  • To transitional housing: 1 point
From transitional housing to unlicensed, independent living: 3 points

​Oregon State Hospital: Salem or Junction City campus

Licensed Residential Setting: SRTF, RTF, RTH, AFH

Congregate Housing: 2 or more adults in shared housing (NOT Roommates if self-selected) - e.g. Room & Board, Oxford Houses

Non-Congregate Housing AKA Independent Living: Individual apartment/house, Rented Room, Living with family, Living with roommates (IF self-selected)

Transitional Housing: Short-term housing e.g. hotels/motels, shelter/mission, repsite car​
