Nonmedical Vaccine Exemptions

How to claim a nonmedical exemption

There are two ways to claim a nonmedical exemption.

  • Option 1: Watch an online education module and submit a certificate of completion.

OR

  • Option 2: Talk to a health care provider and have the practitioner sign a Vaccine Education Certificate. See below for more details.

Option 1: Online education module

IMPORTANT! Read before you start the module:

To protect your privacy, we do not store any information you enter. For this reason, you will need to start over if you close the module before you are finished. If you need to take a break, leave the module window open. When you reach the end, make sure you print your Vaccine Education Certificate before leaving that screen. You will not be able to go back and print the certificate later.

  • Allow 15 to 60 minutes to finish this module. The amount of time depends on the number of vaccines for which you are getting an exemption.
  • Make sure your computer or device is connected to a printer before you begin.
  • This module has audio. Use headphones or make sure your speakers are turned on.
Parents / Guardians
  1. Watch the Vaccine Education Module for children in child care, preschool, or K-12 and print out the Vaccine Education Certificate at the end of the module.
  2. Fill out the nonmedical exemption section of the Certificate of Immunization Status form (pdf).
  3. Turn in both forms to your child's school or child care.

Option 2: Talk to your health care provider

Parents / Guardians

  1. After talking with your health care provider, s/he can give you a signed Vaccine Education Certificate.
  2. Fill out the nonmedical exemption section of the Certificate of Immunization Status form (pdf).
  3. Turn in both forms to your child's school or child care.
College Students
  1. After talking with your health care provider, s/he can give you a signed Vaccine Education Certificate.
