Report Forms
Financial Status Report
Due dates for reports are listed in your grant contract.  Please contact Ferol Weyand by phone at (503) 378-2525 or by email at ferol.weyand@state.or.us if you need a copy of your contract.  Financial reports require an original signature and should be mailed. 
 

Cumulative Grant Activities Report
Grant activities reports may be submitted in paper form or emailed as a Word document.  The Oregon State Library asks that a copy of the last activity report be emailed to be posted on the Web later.  Change the Word table in section 2 to fit your project's outputs and measurable objectives from the grant application.   Call Ferol Weyand in Library Development Services for assistance.


Final Grant Activities Report
The last quarter of a grant has a different report form from the quarterly report.  The focus of the Final Grant Activities Report is on the outcome(s) that the proposal said it would measure.  The Final Grant Activities Report is very analytical and forms the basis of the report that the Institute of Museum and Library Science (IMLS) uses to present information to Congress and national policy makers.

Grant Modification Request
Please submit two copies with original signatures.  We will return one to you to be kept with your copy of the grant contract.

 
Grant Property Reports
This report is only for single items with a life of more than one year and a individual cost of $5,000 or more.

Administrative Requirements
Regulations for managing LSTA grants are found in the Code of Federal Regulations (CFR), and in Office of Management and Budget Circular A-133 which has been codifed into the United States Code (USC).

 
Allowable / Unallowable Costs
Certain types of expenditures are unallowable with Federal Funds.  Regulations from the Office of Management and Budget (OMB) have been codified to Unites States Code.

All required certifications required by the INstitute of Museum and Library Services have been combined into one document

Publicity
Send two copies of all materials produced to the State Library.  Please remember that all promotional materials, press releases, bibliographies, reports and other such publications should have the following acknowledgment:
