Custom mixed feed labeling
Custom mixed feed requirements
After over a year of stake holder input ODA updated the administrative rules concerning custom feed labeling in August 2011. The following pages contain examples illustrating the updated format.
 
A custom mixed feed is a formula sold to only one final consumer. That consumer must be a feeder of animals. Custom mixed feeds are exempt from product registration. However the feed mill producing custom mixed feeds must obtain a commercial feed license.
 
Custom feeds are typically labeled on the invoice, or delivery ticket. The required information must be delivered with the feed.  Bags of custom feed need to have a tag on each bag indicating the manufacturer, Delivery date and Customer name or order number.
 
Shipping documents for custom mixed feeds must include the following information:
  • The name and mailing address of the manufacturer;
  • The name and address of the purchaser;
  • The date of sale or delivery;
  • The custom mixed feed name and brand name if any;
  • If intended for Dairy or swine it shall also bear the guaranteed analysis as stipulated in OAR           603-058-0130(4);
  • The product name and net quantity of each registered commercial feed and each other ingredient used in the mixture;
  • The directions for use and precautionary statements as required by OAR 603-058-0130(6)
  • If a drug containing product is used:
(A)     The Purpose of the medication (claim Statement);
(B)      The established name of each active drug ingredient and the level of each drug used in the final mixture expressed in accordance with OAR 603-058-0130(2).
(i)        The statement: "This feed is formulated for " insert name of final consumer". No resale to other users is allowed."
 
Only custom dairy and swine feeds must also include a guaranteed analysis section.

Caution:  Do not allow horses or other equines access to feed contatining Monensin, Ingestion of Monensin by horses has been fatal.  Monensin medicated cattle and goat feeds are safe for use in cattle and goats only.  Consumption by approved species may result in toxic reactions.  Do not exceed the levels of Monensin recommended in the feeding directions, as reduced average daily gains may result.
 
WARNING:  Do not feed to lactating goats.
 
Feeding Directions: Feed free choice to growing goats.  Provide fresh water.
