At its heart, this model of care fosters strong relationships with patients and their families. Clinics improve care by catching problems earlier, focusing on prevention, wellness and management of chronic conditions.

    Become Recognized: As a recognized primary care home, your clinic will be part of Oregon's new direction to change the way we deliver care to improve health and lower costs.

    Standards for Recognition: Recognized primary care homes are accessible, accountable and offer comprehensive, continuous and coordinated care to patients and their families.

    Payment Incentives: Recognized primary care homes are eligible to receive payment incentives to support the quality, coordinated care they provide Oregonians.
