Traditional Health Workers Certification Applications

All Traditional Health Workers (community health workers (CHW), peer support specialists (PSS), peer wellness specialists (PWS), personal health navigators (NAV), birth doulas) who wish to be certified by the Oregon Health Authority (OHA) must submit a completed application and all necessary documents, including a color copy of Driver's License, or Photo ID, or Passport, to OHA.

The names and contact information of certified Traditional Health Workers (THWs) will be entered into a registry maintained by OHA. Only services provided by certified THWs enrolled in the OHA registry are eligible to be funded by Medicaid.

Application Process

Warning: Registry Enhancements! If you submit your application on this registry it may not get processed!

THW Registry is currently being enhanced. Please submit your application and supporting documents to thw.program@state.or.us. Thank you for your patience.

Individuals can still find a THW or Doula in their area by clicking on the registry link below, clicking on the Search tab then selecting THW or Doula in the Narrow Your Search drop down menu.

Please create an account and submit your application on the Traditional Health Worker Registry:

https://traditionalhealthworkerregistry.oregon.gov/

In order for your application to be deemed complete, you must upload a copy of your diver's license along with proof of training program completion.

See "Application Type" section below to determine which type of application you should submit.

If you prefer to submit a paper application, you may download a form below.

Background Check

Once OEI receives your completed application and supporting documents, your information will be sent to the Background Check Unit. You will receive an e-mail asking you to complete a Background Check application. You must complete this for your application process to move forward. Please note that if you have lived outside of Oregon in the past 5 years, you will be asked to submit fingerprints.

You may find more information about the background check here: THW Background Check, Weighing Test

If OHA determines that an applicant has met all requirements, including passing a background check, OHA will notify the applicant in writing of its decision to grant certification or provisional certification as a THW. The applicant’s name and contact information will be added to the registry of certified THWs.
