Traditional Health Workers Certification Applications

All Traditional Health Workers (community health workers (CHW), peer support specialists (PSS), peer wellness specialists (PWS), personal health navigators (NAV), birth doulas) who wish to be certified by the Oregon Health Authority (OHA) must submit a completed application and all necessary documents, including a color copy of Driver's License, or Photo ID, or Passport, to OHA.

The names and contact information of certified Traditional Health Workers (THWs) will be entered into a registry maintained by OHA. Only services provided by certified THWs enrolled in the OHA registry are eligible to be funded by Medicaid.

Individuals can still find a THW or Doula in their area by clicking on the registry link below, clicking on the Search tab then selecting THW or Doula in the Narrow Your Search drop down menu.

In order for your application to be deemed complete, you must upload a copy of your diver's license along with proof of training program completion.

See "Application Type" section below to determine which type of application you should submit.

Background Check 

Once OEI receives your completed application and supporting documents, your information will be sent to the Background Check Unit. You will receive an e-mail asking you to complete a Background Check application. You must complete this for your application process to move forward. Please note that if you have lived outside of Oregon in the past 5 years, you will be asked to submit fingerprints.

If OHA determines that an applicant has met all requirements, including passing a background check, OHA will notify the applicant in writing of its decision to grant certification or provisional certification as a THW. The applicant’s name and contact information will be added to the registry of certified THWs.

Application Type

Full Certification 

Complete this application​ if you meet the following:

  • Successfully completed all required training for one of the worker types (Birth Doula, CHW, PSS, PWS, NAV) through an OHA-approved training program

Full Certification Through Grandfathering 

Complete the this application​ application if you meet all of the following: 

  • Worked or volunteered in the capacity of a CHW, PWS, or NAV in the state of Oregon at least 3000 hours in the past five (5) years from the date of this application for certification; AND 
  • Successfully completed incumbent worker training offered by an OHA-approved training program

Provisional Certification (Temporary, for 1 year)

Complete this application​ if you meet one of the following: 

  • Successfully completed any non-approved THW training program between February 4, 2010 to August 2, 2013; AND
  • The program you completed included a minimum of 40 contact hours of training


  • Are eligible for grandfathering as described above but have not yet completed incumbent worker training

Renewal for Fully Certified THWs

Complete this application​ if you meet the following:

  • Completed at least 20 hours of continuing education 

​Note: You must apply for renewal every 3 years and submit a renewal application no less than 30 days prior to the expiration of your current certification period. The renewal timeline for THWs who have been issued certification for both "THW" and "THW:Birth Doula" on different dates will be synced to the certification period that expires first.
