Oregon Yellow Fever Program Information

Certification, Stamp Approval, and Administration Site Registration

Providers interested in administering yellow fever vaccine need to apply as a yellow fever provider to receive a registered yellow fever stamp.

The Yellow Fever stamp is issued for authentication of International Certificates of Vaccination or Prophylaxis (ICVP). It allows the certified provider to sign this “yellow card”, validating the record. The registration of yellow fever administration sites allows providers to purchase and store yellow fever vaccine and supplies.

Guidance on reporting vaccination errors is available if you have additional questions. If you need further assistance with reporting to VAERS, please email info@VAERS.org or call 1-800-822-7967.
