Columbia Pacific Coordinated Care Organization, Tillamook County Health and Human Services, and Tillamook Family Counseling Center are working together to improve the quality, delivery, and accessibility of health care in Tillamook County. Collaborative efforts have focused on advancing a model of physical and behavioral health care that fully integrates primary care, behavioral health providers, and public health resources to provide holistic, community-based care.

Trillium Community Health Plan CCO and Lane County Health and Human Services

Trillium Community Health Plan CCO and Lane County Health and Human Services are working together to improve the health of residents in Lane County, Oregon by uniting the work of physical and behavioral health providers and implementing population-based programs to address tobacco use and obesity in low income communities.

Yamhill Community Care Organization and Yamhill County Health and Human Services

Yamhill Community Care Organization and Yamhill County Health and Human Services are working together to improve the quality, delivery, and accessibility of health care in Yamhill County by collaborating on a new early learning hub, streamlining referrals to early childhood home visiting services, implementing a maternal medical home and leveraging an adult community health worker hub to serve community members with complex chronic conditions.

Central Oregon Health Council Maternal and Child Health Initiative

The Crook, Deschutes and Jefferson county health departments are working with PacificSource Central Oregon Coordinated Care Organization and several other local organizations to implement a maternal and child health care coordination system across the tri-county region. The maternal and child health initiative expands care coordination and home visiting programs to better meet the needs of pregnant women and children and improve health outcomes.
