Renew My License or Certificate

What Type of License Do You Need?

If you are currently (or have been) an Oregon nurse or nursing assistant, please review the application options below.  You can renew or reactivate your current or expired Oregon license/certificate, or reinstate licenses/certificates that have been revoked or voluntarily surrendered.

​If your Oregon nursing license or nursing assistant certificate is active, or has been expired fewer than 60 days, you may apply to renew your license or certificate.

What happens next:  Once we receive your completed application and payment, we will begin processing your application.  You will receive an email from us once your license/certificate has been renewed.

Refund Policy:  You may pay by credit card or check.  Fees submitted as part of the application process are not refundable, per ORS 678.410(2).  Be certain that you intend to complete the process before submitting your payment.  If you have any questions regarding your eligibility, or there is an error in your payment, please call our Customer Service Center at 971-673-0685 or e-mail oregon.bn.info@state.or.us.

​If your Oregon nursing license or nursing assistant certificate has been expired for 61 days or more, you may apply to reactivate (please see applications for specific requirements) your license or certificate:

  • You are a RN or LPN who can meet the practice requirement of 960 hours of nursing practice within the last five years.  Please review our helpful tutorial on how to complete the application.
  • You are a nurse practitioner who:
    • meets the practice requirement of 960 practice hours in the NP specialty you hold and has active national certification in that same NP specialty, or
    • was licensed prior to January 1, 2011, and you have completed 45 hours of continuing education (CE) with 15 hours being in pharmacological content.
  • You are a CRNA who has active unencumbered national certification.
  • You are a CNS who meets the practice requirement of 960 practice hours, and has completed 45 hours of continuing education (15 hours of which is pharmacological content), or has active CNS national certification.
  • You are a CNA who either completed a training program within the last two years, or you can meet the work requirement of having at least 400 paid work hours (doing CNA authorized duties under the supervision or monitoring of a RN) in the past two years.  If you are a student nurse, a nurse who has graduated within the last year, or licensed LPN/RN that is wanting to reactivate your expired CNA certification, see the application for details.
  • You are a CMA who meets the work requirement of having at least 400 paid work hours doing CMA authorized duties under the supervision or monitoring of a RN in the past two years, and has completed eight hours of CE in pain management before your certification expired.  If you did not have 400 CMA work hours or did not complete the CE hours before your certification expired, you must retake and pass the CMA examination to reactivate

Reactivation Fees and Refund Policy

  • CNA reactivation: $69.
  • CNA/CMA reactivation: $84
  • RN/LPN reactivation: $264.  (View a tutorial on how to complete the application​.)
  • RN/APRN reactivation with no prescriptive authority: $419
  • RN/APRN reactivation with prescriptive authority: $519
  • You may pay by credit card or check.  Fees submitted as part of the application process are not refundable, per ORS 678.410(2).  Be certain that you intend to complete the process before submitting your payment.  If you have any questions regarding your eligibility, or there is an error in your payment, please call our Customer Service Center at 971-673-0685 or e-mail oregon.bn.info@state.or.us.

​If your Oregon nursing license has been expired for 61 days or more, and you cannot meet the practice hour requirement (960 hours within five years), you must complete an a nurse re-entry program while under limited licensure in order to be eligible to reactivate your license.

​If your CNA certificate has been expired 61 – 730 days, and you cannot meet the work hour requirement, you must apply to reactivate your CNA by taking and passing the CNA competency examination​.

What happens next:  After you submit your application and payment, you will receive:

  1. An email from the OSBN with instructions on how to complete the fingerprinting process for your national criminal background check.  More information about Oregon’s digital fingerprint system: Frequently Asked Questions about fingerprinting.
  2. An email from Headmaster Testing Service with instructions on how to schedule your exam date.  Headmaster will send this email after the OSBN reviews your application and releases you to test.  Once you choose a date, Headmaster will send you a confirmation email.

Note for Applicants:

  • Headmaster Testing Service provides Oregon’s CNA and CMA testing.  All applicants should review the Candidate Handbook for Oregon before submitting their application.
  • Exam results are available on Headmaster’s website 3-5 business days after the exam date. 
  • If you pass, and you have met all other requirements for certification, the OSBN will reactivate your Oregon CNA certification.  If you do not pass, your results online will indicate which skills or areas need improvement, and instructions on how to schedule another exam. 

CNA Exam Fees and Refund Policy

  • Reactivate CNA Certification via Examination:  $106.
  • Manual skills exam retest: $45.
  • Written exam retest: $25.
  • Oral exam: $35

What happens next:  After you submit your application and payment, you will receive:
