The Board of Nursing meets 12 times a year and takes disciplinary actions during each meeting.  Listed below are the meetings held for the past year.  The status and any disciplinary documents for individual nurses or nursing assistants licensed in Oregon are available on the OSBN Verification System.  Or, you can download a file of all actions taken during the meetings below.
 
​​The Oregon State Board of Nursing (OSBN) took final disciplinary action against the following nurses or nursing assistants during its August 17, 2022, Board Meeting.  The status of individual nurses or nursing assistants licensed in Oregon is available on the OSBN Verification System​​.  Or, you can download a file of all actions​ taken during the meeting. ​

Benton County
  • The Board reprimanded the registered nurse license of Herma Ornes (200242299RN) for failing to administer medications in a manner consistent with state and federal law, abusing a client, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Corvallis, Ore.
Clackamas County
  • The Board accepted the Voluntary Surrender of Margaret A. Griffith’s registered nurse license (083043001RN) for violating the terms and conditions of a Board Order.  She resides in Lake Oswego, Ore.
  • The Board denied the CNA application of Sophie M. Padilla for failing to cooperate during an investigation.  She resides in Milwaukie, Ore. 
Coos County
  • The Board imposed a $1,600 civil penalty on Pamela L. Inman’s practical nurse license (083044914LPN) for practicing nursing without a current Oregon license.  She resides in Coos Bay, Ore.
Crook County
  • The Board reprimanded the nursing assistant certificate of Kimberly D. Baker (202108596CNA) for failing to respect client dignity and rights.  She resides in Prineville, Ore.
Deschutes County
  • The Board reinstated and placed the registered nurse license of Stacey L. Boeholt (201242364RN) on probation for 24 months for practicing nursing while impaired.  She resides in Bend, Ore.
  • The Board placed the registered nurse license of Kimberly A. Owen (200141403RN) on probation for 24 months for practicing nursing while impaired and using intoxicants in a manner dangerous to herself or others.  She resides in Bend, Ore.
Jackson County
  • The Board denied the CNA application of Leann R. Morris for misrepresentation in applying for a certificate.  She resides in Eagle Point, Ore.
  • The Board revoked the practical nurse license of Brittany P. Oliver (202008176LPN) for violating the terms and conditions of a Board Order and failing to cooperate during an investigation.  She resides in Medford, Ore.
Lane County
  • The Board suspended the nursing assistant certificate of Lyndsay L.A. Pique (201606954CNA) for a minimum 14 days for failing to cooperate with the Board during an investigation.  She resides in Cottage Grove, Ore.

  • The Board placed the registered nurse license of Kyla R.   • The Board accepted the voluntary withdrawal of Aubrey J. Comstock’s RN reinstatement application for using intoxicants in a manner dangerous to herself or others.  She resides in Portland, Ore.

Multnomah County
  • The Board placed the registered nurse license of Rachel E. Beyer (098003060RN) on probation for 12 months with conditions for neglecting a client, falsifying data, and failing to conform to the essential standards of nursing practice.  She resides in Portland, Ore.
Umatilla County
  • The Board placed the registered nurse license of Elizabeth A. Johnston (200940201RN) on probation for 24 months for neglecting a client and failing to conform to the essential standards of acceptable nursing practice.  She resides in Pendleton, Ore.
Washington County
  • The Board placed the registered nurse license of Trisha J. Mannix (200942239RN) on probation for 24 months for failing to comply with the terms and conditions of the Health Professionals’ Services Program.  She resides in Hillsboro, Ore.

  • California—The Board imposed a $2,500 civil penalty and suspended the practical nurse license of Sara A. Cabitto (201605915LPN) for 12 months for violating the terms and conditions of a Board Order, demonstrated incidents of dishonesty, and entering inaccurate documentation into a health record.  She resides in Yreka, Calif.
  • California—The Board accepted the voluntary withdrawal of Edemel E. Pareno’s RN application for resorting to deceit during the application process for licensure.  He resides in Union City, Calif.
  • California— The Board reinstated and placed the practical nurse license of Mindy R. Thomas (201402014LPN) on probation for 12 months due to previous revocation of her license.  She resides in Yreka, Calif.
  • Idaho— The Board suspended the CRNA license of Bradley S. Dalling (201811292CRNA) for 12 months, to be followed by two years of probation, and placed his RN license (201811282RN) on probation until his CRNA suspension and probation is completed.  For falsifying data, using intoxicants to the extent injurious to himself or others, obtaining unauthorized medications, and entering inaccurate documentation into a health record.  He resides in Fruitland, Idaho.
  • Idaho—The Board reprimanded and imposed a $500 civil penalty on Edward L. Dietrich’s registered nurse license (201340634RN) for the unauthorized removal of client information from the workplace and conduct derogatory to the standards of nursing.  He resides in Kuna, Idaho.
  • New Mexico— The Board revoked the registered nurse license of Dorothy A. Roman (201341736RN) for demonstrated incidents of dishonesty, falsifying data, and failing to cooperate with the Board during an investigation.  She resides in Las Vegas, N.M.
  • Texas—The Board accepted the voluntary withdrawal of Chester Jones IV’s RN application for failing to answer questions truthfully on an application for licensure.  He resides in Irving, Texas.

Clackamas County
  • The Board reprimanded the practical nurse license of Barbara Mickel (200930361LPN) and imposed a $700 civil penalty for violating a person’s rights of privacy and confidentiality by accessing information without proper authorization or a demonstrated need to know.  She resides in Estacada, Ore.
  • The Board accepted the Voluntary Surrender of Chayla J. Owen’s nursing assistant certificate (201710598CNA) for failing to respect the dignity and rights of clients, and demonstrated incidents of abusive behavior.  She resides in Canby, Ore.
Columbia County
  • The Board denied Amy A. Pritchett’s nursing assistant renewal application (200711239CNA) for failing to answer application questions truthfully and failing to cooperate with the Board during an investigation.  She lives in Saint Helens, Ore.
