The Board of Nursing meets 12 times a year and takes disciplinary actions during each meeting.  Listed below are the meetings held for the past year.  The status and any disciplinary documents for individual nurses or nursing assistants licensed in Oregon are available on the OSBN Verification System.  Or, you can download a file of all actions taken during the meetings below.

 

The Oregon State Board of Nursing (OSBN) took final disciplinary action against the following nurses or nursing assistants during its June 12, 2019, Board Meeting.  The status of individual nurses or nursing assistants licensed in Oregon is available on theOSBN Verification System.  Or, you can download a file of all actions taken​ during the meeting.

Benton County

  • The Board accepted the voluntary withdrawal of Stevon L. McBride’s CNA application for misrepresentation during application for certification and impairment.  He resides in Corvallis, Ore.

Clackamas County

  • The Board suspended the nursing assistant certificate of Robert Espinosa (000003872CNA) for a minimum of 14 days for failing to cooperate with the Board during an investigation.  He resides in Milwaukie, Ore.
  • The Board placed the registered nurse license of Jason R. Gilliland (201602606RN) on probation for 24 months for use of intoxicants to the extent injurious to himself or others.  He resides in Happy Valley, Ore.

Clatsop County

  • The Board placed the registered nurse license of Scott T. Brownlee (201904618RN) on probation for 24 months for use of intoxicants to the extent injurious to himself or others.  He resides in Seaside, Ore.

Columbia County

  • The Board reprimanded the registered nurse license of Mark A. Smith (092005128RN), of Scappoose, Ore., for untimely and incomplete documentation, and failing to conform to the essential standards of acceptable nursing practice.

Coos County

  • The Board placed the registered nurse license of Bobbie M. Felton (200942261RN) on probation for 12 months for failure to accurately document nursing interventions and practice in a timely manner, and use of intoxicants to the extent injurious to herself or others.  She resides in Coquille, Ore.
  • The Board accepted the Voluntary Surrender of Janis M. Petrie’s registered nurse, clinical nurse specialist, and family nurse practitioner licenses (200440305RN/ 201070011CNS-PP/ 200450014NP) for failing to release a client’s health record within 60 days from receipt of written notice for release, failing to communication client status information to other authorized individuals, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Bandon, Ore.
  • The Board suspended the registered nurse license of Crystal D. Whited (201243520RN) for a minimum of 14 days for failing to cooperate with the Board during an investigation.  She resides in Coos Bay, Ore.

Deschutes County

  • The Board accepted the Voluntary Surrender of Thomas L. Martin’s registered nurse license (201400886RN) for failing to comply with the terms and conditions of the Health Professionals’ Services Program.  He resides in Bend, Ore.

Douglas County

  • The Board reprimanded the registered nurse license of Gary L. Fuller (090003311RN), of Reedsport, Ore., for assigning persons to perform functions for which they are not prepared to perform and beyond their authorized duties, and failing to conform to the essential standards of acceptable nursing practice.

Jackson County

  • The Board placed the nursing assistant certificate of Vadra M. Bezner (201904616CNA) on probation for 24 months for conviction of a crime that bears demonstrable relationship to CNA duties and impairment.  She resides in Medford, Ore.

Lane County

  • The Board placed the nursing assistant certificate of Lauren A. Ellis (201904631CNA) on probation for 24 months for use of intoxicants to the extent injurious to herself or others.  She resides in Springfield, Ore.
  • The Board reprimanded the registered nurse license of Jennifer R. Lavassaur (201705259RN), of Eugene, Ore., for failing to respect the dignity and rights of clients.
  • The Board placed the practical nurse license of Thomas E. Mulcahy (201505672LPN) on probation for 24 months for use of intoxicants to the extent injurious to himself or others.  He resides in Eugene, Ore.
  • The Board placed the practical nurse license of Marisia A. Ybarra (201601737LPN) on probation for 24 months for performing acts beyond her authorized scope, and use of intoxicants to the extent injurious to herself or others.  She resides in Eugene, Ore.

Linn County

  • The Board suspended the practical nurse license of Patrick K. House (201702155LPN) for 300 days (retroactively) for conviction of a crime that bears a demonstrable relationship to nursing, and demonstrated incidents of reckless behavior.  He resides in Albany, Ore.
  • The Board accepted the Voluntary Surrender of Tiffany A. Sanders’s nursing assistant certificate (201605901CNA) for failing to maintain professional boundaries, and using her role as a CNA for personal gain.  She resides in Albany, Ore.

Lincoln County

  • The Board denied the CNA application of Amy G. Garrett, of Logsden, Ore., for failing to answer questions truthfully and demonstrated incidents of dishonesty.

Marion County

  • The Board placed the nursing assistant certificate of Todd A. Boone (201393498CNA) on probation for 24 months for use of intoxicants to the extent injurious to himself or others.  He resides in Salem, Ore.
  • The Board suspended the practical nurse license of Jill C. Flanders (201700229LPN) for seven days, to be followed by 12 months of probation, for failing to report suspected incidents of abuse, incomplete and untimely documentation, failing to answer questions truthfully, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Salem, Ore.
  • The Board placed the registered nurse license of Danyelle H. Landry (200641652RN) on probation for 24 months for obtaining unauthorized medications, and use of intoxicants to the extent injurious to herself or others.  She resides in Salem, Ore.
  • The Board accepted the Voluntary Surrender of Anne M. Meeks’s registered nurse license (200940236RN) for violating the terms and conditions of a Board Order.  She resides in Keizer, Ore.
  • The Board placed the registered nurse license of Christine E. Rivera (200841405RN) on probation for 24 months for failing to comply with the terms and conditions of the Health Professionals’ Services Program.  She resides in Salem, Ore.
  • The Board suspended the registered nurse license of Brenda J. Torres (200941914RN) for 30 days for failing to failing to clinically supervise a UAP to whom a nursing procedure had been delegated, neglecting a client, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Keizer, Ore.

Multnomah County

  • The Board placed the registered nurse and family nurse practitioner licenses of Theresa L. Bunker (096006685RN/096006685N1) on probation for 12 months for failing to respect the dignity and rights of clients, and engaging in other unacceptable behavior towards or in the presence of a client.  She resides in Portland, Ore.
  • The Board reprimanded the certified registered nurse anesthetist license of Verity A. Caruso (201500622CRNA) for failing to take action to preserve client safety, failing to communicate information regarding client status to members of the healthcare team, and failing to conform to the essential standards of acceptable nursing practice.
  • The Board accepted the Voluntary Surrender of Monique Jefferson’s nursing assistant certificate (201111529CNA) for abusing a person, failure to report her felony conviction within 10 days, and failing to answer questions truthfully.  She resides in Portland, Ore.
  • The Board suspended the registered nurse license of Diane L. Panian (083039818RN) for three months, to be followed by 12 months of probation, for failing to respect the dignity and rights of clients, and demonstrated incidents of reckless behavior.  She resides in Portland, Ore.

Tillamook County

  • The Board reprimanded the nursing assistant certificate of Machala S. Loving (201707234CNA), of Tillamook, Ore., for failing to implement the plan of care developed by the RN.

Washington County

  • The Board reprimanded the practical nurse license of Erica M. Divine (201230646LPN), of Beaverton, Ore., for leaving a nursing assignment without confirming that nursing assignment responsibilities will be met.
  • The Board accepted the Voluntary Surrender of Kimberlee A. Dixon’s registered nurse license (082012285RN) for demonstrated incidents of dishonesty and failing to conform to the essential standards of acceptable nursing practice.  She resides in Tigard, Ore.

Out of State

  • California— The Board suspended the nursing assistant certificate of Concepcion P. Armstrong (201608916CNA) for a minimum of 14 days for failing to cooperate with the Board during an investigation.  She resides in Crescent City, Calif.
  • Florida— The Board denied the CNA application of Lawrence T. Francis, of Jacksonville, Fla., for misrepresentation during the application process for certification and failing to cooperate with the Board during the course of an investigation.
  • South Carolina— The Board suspended the registered nurse license of Sarah M. Webber (201701655RN) for a minimum of 14 days for failing to cooperate with the Board during an investigation.  She resides in Greenville, S.C.
  • Virginia— The Board suspended the registered nurse license of Pepito J. Decena (201901069RN) for a minimum of 14 days for failing to cooperate with the Board during an investigation.  She resides in Sterling, Va.
  • Washington— The Board accepted the Voluntary Surrender of Jered W. Brannan’s practical nurse license (201230287LPN) for willful misrepresentation in applying for a license renewal.  He resides in Camas, Wash.
  • Washington— The Board accepted the Voluntary Surrender of Theresa M. Hines’s family nurse practitioner license (200850166NP) for failing to comply with the terms and conditions of the Health Professionals’ Services Program.  She resides in Vancouver, Wash.
  • Wisconsin— The Board accepted the Voluntary Surrender of Joanne M. Rutland’s registered nurse, psychiatric/mental health nurse practitioner, and dispensing licenses (099000440RN/ 200150043NP/ 201400760DP) due to a physical condition that prevents her from practicing safely.  She resides in Middleton, Wis.

Clackamas County
  • The Board suspended the nursing assistant certificate of Eugene P. Kuntz (201501462CNA) for a minimum of 14 days for failing to cooperate with the Board during an investigation.  He resides in West Linn, Ore.
  • The Board reprimanded the registered nurse license of Mark G. Petitmermet (201241724RN), of Oregon City, Ore., for failing to communicate information regarding client status to members of the healthcare team, and failing to conform to the essential standards of acceptable nursing practice.
Coos County
  • The Board revoked Ashley M. Webber’s registered nurse license (201803842RN) for obtaining unauthorized drugs from the workplace, misrepresentation during the licensure process, and failing to answer questions truthfully.  She resides in Coos Bay, Ore.
Deschutes County

Douglas County

  • The Board reprimanded the registered nurse and family nurse practitioner licenses of Mary L. Hagood (200240641RN/200250023NP) for failing to administer medications in a manner consistent with state and federal law, implementing standards of practice that jeopardize patient safety, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Roseburg, Ore.

Wasco County
  • The Board accepted the Voluntary Surrender of Jason A. Haugen’s nursing assistant and medication aide certificates (000036414CNA/ 200020112CMA) for obtaining unauthorized medications, the unauthorized removal of drugs from the workplace, and failing to fully cooperate with the Board during an investigation.  He resides in The Dalles, Ore.

  • Colorado— The Board denied the LPN application of Chamelon Jackson, of Aurora, Colo., for willful misrepresentation in applying for a license, and failing to cooperate fully with the Board during an investigation.
  • Illinois—The Board placed the registered nurse license of Michael F. Cottrell (201902795RN) on probation for 24 months due to previous discipline in Illinois.  He resides in Saint Charles, Ill.
  • Washington— The Board suspended the registered nurse license of Eric L. Burke (200242017RN) for a minimum of 14 days for failing to cooperate with the Board during an investigation.  He resides in Battle Ground, Wash.
  • Washington— The Board reprimanded the family nurse practitioner and registered nurse licenses of Bradley W. Hilliard-Lythgoe (201800005RN/ 201800006NP-PP), of Spokane, Wash., for failing to accurately document nursing interventions and practice.

  • The Board reprimanded the registered nurse license of David A. Ritter (200541115RN), of West Linn, Ore., for failing to respect a client’s dignity and rights, engaging in abusing behavior toward a coworker, and violating a client’s rights of privacy and confidentiality.
