The Board of Nursing meets 12 times a year and takes disciplinary actions during each meeting.  Listed below are the meetings held for the past year.  The status and any disciplinary documents for individual nurses or nursing assistants licensed in Oregon are available on the OSBN Verification System.

Clackamas County
  • The Board placed the registered nurse license of Bridgette N. Beliz (096000111RN) on probation for 24 months for using intoxicants to the extent injurious to herself or others and practicing while impaired.  She resides in Milwaukie, Ore.
  • The Board reprimanded the registered nurse license of Alexsandra A. Thomas (202107597RN) for failing to administer medications in a lawful manner and failing to accurately document nursing practice implementation.  She resides in Happy Valley, Ore.

Jackson County

  • The Board suspended the registered nurse license of Lisabeth J. Henshaw (089003140RN) for 30 days for failing to be respectful to the client's family.  She resides in Talent, Ore.
  • The Board reprimanded the nursing assistant certificate of Sheila Waara (202001222CNA) demonstrated incidents of abusive behavior and dishonesty.  She resides in Medford, Ore.
  • The Board revoked Jennifer F. Wilkerson's practical nurse license (201030447LPN) for the unauthorized removal of medications from the workplace and practicing nursing while impaired.  She resides in Central Point, Ore.
Marion County
  • The Board reprimanded the registered nurse license of Tia L. Dobson (200341149RN) with conditions for performing acts beyond her authorized scope of practice and failing to conform to the essential standards of acceptable nursing practice.  She resides in Salem, Ore.
  • The Board suspended the nursing assistant certificate of Nancy E. Parrott (000032554CNA) for a minimum of 14 days for failing to cooperate with the Board during an investigation.  She resides in Keizer, Ore.
  • The Board placed the practical nurse license of Carissa A. Teacutter (201908065LPN) on probation for 24 months for using intoxicants to the extent injurious to herself or others.  She resides in Salem, Ore. 
Multnomah County
  • The Board suspended the registered nurse and certified registered nurse anesthetist licenses of Bradley D. Burch (201503211CRNA,200642817RN) for 12 months, to be followed by 24-months of probation, for falsifying data and obtaining unauthorized medications.  He resides in Portland, Ore.
  • The Board reprimanded the registered nurse license of Hope C. Garcia (202101277RN) and imposed a $2,400 civil penalty for violating a person's rights to privacy and confidentiality.  She resides in Portland, Ore.
Washington County
  • The Board accepted the Voluntary Surrender of Julia N. Rodriguez's regi​stered nurse license (200840519RN) for violating the terms and conditions of a Board Order.  She resides in Hillsboro, Ore.
Out of State
  • Alabama—The Board accepted the Voluntary Surrender of Shaun D. Tolbert's registered nurse license (201905636RN) for violating the terms and conditions of a Board Order.  He resides in Homewood, Ala.
  • Colorado— The Board accepted the Voluntary Surrender of Beth C. Blume's registered nurse license (201905859RN) for failing to provide the Board with requested documents.  She resides in Colorado Springs, Colo.

  • The Board imposed a $500 civil penalty against the registered nurse license of Kristy N. Lomax (200140166RN) for practicing nursing without a current Oregon license.  She resides in Scappoose, Ore.

  • The Board revoked the nursing assistant certificate of Tonya L. Marino (000044589CNA) for demonstrated incidents of dishonesty and falsifying data.  She resides in Roseburg, Ore.
Jackson County
  • The Board suspended the practical nurse license of Marie I. Johnson-Blount (201703975LPN) for a minimum of 14 days for failing to cooperate with the Board during an investigation.  She resides in Central Point, Ore.
Josephine County
  • The Board reprimanded the nursing assistant certificate of Davina M. Mooney (201810621CNA) and imposed a $500 civil penalty for violating a person's rights to privacy and confidentiality.  She resides in Merlin, Ore. 
Lane County
  • The Board accepted the Voluntary Surrender of Jene M. Wittman's nursing assistant and medication aide certificates (200820082CMA/200510451CNA) for the unauthorized removal of medications from any setting and diverting medication for use by herself or others.  She resides in Florence, Ore. 
Marion County
  • The Board suspended the practical nurse license of Rebecca E. Garza (201609512LPN) for 30 days, to be followed by 24-months of probation, for failing to maintain professional boundaries with a client, engaging in sexual contact with a client in any setting, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Keizer, Ore.
  • The Board accepted the Voluntary Surrender of Susan M. Huggins's registered nurse license (096006628RN) for failing to comply with the terms and conditions of the Health Professionals' Services Program.  She resides in Woodburn, Ore.

  • Texas—The Board accepted the voluntary withdrawal of Joe J. Vidal's RN application due to a current probation of his Texas license.  He resides in Midland, Texas.

  • The Board reprimanded the registered nurse license of Kimberly A. Gilder (201141857RN) for conduct derogatory to the standards of nursing.  She resides in Redmond, Ore.
Hood River County
  • The Board placed the registered nurse license of Julianna Mears (092007235RN) on probation for 24 months after completing re-entry due to a previous license revocation.  She resides in Hood River, Ore.

  • The Board reprimanded the practical nurse license of June I. D.
