The Board of Nursing meets 12 times a year and takes disciplinary actions during each meeting.  Listed below are the meetings held for the past year.  The status and any disciplinary documents for individual nurses or nursing assistants licensed in Oregon are available on the OSBN Verification System.  Or, you can download a file of all actions taken during the meetings below.
 
​​The Oregon State Board of Nursing (OSBN) took final disciplinary action against the following nurses or nursing assistants during its March 17, 2021, Board Meeting.  The status of individual nurses or nursing assistants licensed in Oregon is available on the OSBN Verification System.  Or, you can download a file of all actions taken during the meeting.

Clackamas County
  • The Board suspended the practical nurse license of Jennifer L. Alvarado (200630431LPN) for a minimum 14 days for failing to cooperate with the Board during the course of an investigation.  She resides in Oregon City, Ore.
  • The Board suspended the nursing assistant certificate of Patty L. Benedict (200910730CNA) for a minimum 14 days for failing to cooperate with the Board during the course of an investigation.  She resides in Gladstone, Ore.
Clatsop County
  • The Board suspended the nursing assistant certificate of Josiah Byers (201806967CNA) for a minimum 14 days for failing to cooperate with the Board during the course of an investigation.  He resides in Hammond, Ore.
  • The Board reprimanded the nursing assistant certificate of Mina Luna V. Ferrer (200611374CNA) for jeopardizing the safety of a client under her care, and failing to take action to preserve a client’s safety.  She resides in Warrenton, Ore.
  • The Board placed the registered nurse license of Teresa M. Martinolich (097003211RN) on probation for 24 months (after she successfully completes a reentry program and other conditions) for unauthorized removal of medications from the workplace, falsifying data, and failing to answer questions truthfully.  She resides in Astoria, Ore.
Coos County
  • The Board accepted the Voluntary Surrender of Christine L. Ulibarri’s registered nurse license (200240976RN) for using intoxicants to the extent injurious to herself or others, and failing to comply with the terms and conditions of the Health Professionals’ Services Program.  She resides in Myrtle Point, Ore.

  • The Board accepted the Voluntary Surrender of Delia A. Anderson’s nursing assistant certificate (200111786CNA) for the unauthorized removal of money from a person, and failing to maintain professional boundaries.  She resides in Roseburg, Ore.

  • The Boar​d suspended the registered nurse license of Alicia D. Hills (200341250RN) for a minimum 14 days for failing to cooperate with the Board during the course of an investigation.  She resides in Baker City, Ore.
Clackamas County
  • The Board revoked the nursing assistant certificate of Hiwot A. Alemu (201810062CNA) for abuse of a person, failing to respect a person's dignity and rights, and failing to answer questions truthfully.  She resides in Happy Valley, Ore.
  • The Board reprimanded the registered nurse license of Kahlei A. Howard (200441643RN) for assigning persons to perform functions for which they are not prepared, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Clackamas, Ore.

  • The Board placed the registered nurse license of Teresa M. Martinolich (097003211RN) on probation for 24 months for possessing unauthorized medications and the unauthorized removal of medications from the workplace.  She resides in Astoria, Ore.

Curry County

  • The Board accepted the Voluntary Surrender of Nancy A. Erb's geriatric nurse practitioner license (200650088NP) and place her registered nurse license (095007161RN) on probation for 12 months for prescribing drugs to an individual who was not her client, failing to establish professional boundaries, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Brookings, Ore.

Deschutes County

  • The Board placed the registered nurse license of Rita M. Shearer (081001191RN) on probation for 24 months for using intoxicants to the extent injurious to herself or others, and failing to report to the Board a conviction within 10 days.  She resides in Bend, Ore.

Jackson County

  • The Board denied the CNA application of Talisha L. Bigelow for conviction of a crime that bears demonstrable relationship to nursing assistant duties.  She resides in Medford, Ore.
  • The Board revoked the registered nurse license of Carolyn M. Schoenfelder (200742872RN) for failing to cooperate with the Board during the course of an investigation.  She resides in White City, Ore.

Josephine County

Lane County

  • The Board accepted the Voluntary Surrender of Jonathan N. Anderson's practical nursing license (202005712LPN) for failing to answer questions truthfully and demonstrated incidents of dishonesty.  He resides in Springfield, Ore.
  • The Board placed the nursing assistant certificate of Fallon T. King (201900337CNA) on probation for six months for demonstrated incidents of neglectful behavior, and failing to conform to the essential standards acceptable CNA performance.  She resides in Eugene, Ore.
  • The Board placed the women's health care nurse practitioner license of Anne M.   • California— The Board placed the nursing assistant certificate of Simon Patfield (202101591CNA) on probation for 12 months for conduct that is substantially related to the fitness and ability of the applicant.  He resides in San Francisco, Calif.
  • Ohio—The Board accepted the Voluntary Surrender of Joshua B. Keyes's registered nurse license (201800361RN) for resorting to misrepresentation during the application process for license renewal.  He resides in Canal Winchester, Ohio.
  • Washington—The Board imposed a $2,000 civil penalty against Kristie M. Andreas's registered nurse license and family nurse practitioner licenses (201509133RN/ 201509134NP-PP) and, when she returns to practice, she will be on probation for 12 months.  The discipline was imposed for failing to answer questions truthfully, incomplete recordkeeping, failing to take action to preserve client safety, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Skamokawa, Wash.
  • Washington— The Board suspended the practical nurse license of Andrea R. Cassa-Brewster (201130437LPN) for a minimum 14 days for failing to cooperate with the Board during the course of an investigation.  She resides in Brush Prairie, Wash.

  • The Board reprimanded the nursing assistant certificate of Tatyana A. Voronova (201807777CNA) for jeopardizing the safety of a person under the CNA’s care, and demonstrated incidents of neglectful behavior.  She resides in Gresham, Ore.
Washington County
  • The Board suspended the certified registered nurse anesthetist license of Aaryo Bakhtiar (202000790CRNA) for 365 days, to be followed by 24 months of probation, and placed his RN license (201404127RN) on immediate probation for 36 months, for the unauthorized removal of medications from the workplace, possessing unauthorized drugs, and demonstrated incidents of reckless behavior.  He resides in Beaverton, Ore.
  • The Board placed the registered nurse and certified registered nurse anesthetist licenses of Daniel A. Jones (200840847RN/ 202003239CRNA-PP) on probation for 18 months for demonstrated incidents of reckless behavior, violating client’s rights to privacy and confidentiality, and using intoxicants to the extent injurious to himself or others.  He resides in Portland, Ore.

  • Georgia— The Board revoked the registered nurse and family nurse practitioner licenses of Alice Minkoff (201908312RN/ 201908360NP-PP) for demonstrated incidents of dishonesty, gross incompetence, and failing to answer questions truthfully.  She resides in Snellville, Ga.
  • Louisiana— The Board reprimanded the registered nurse license of Ray Young (201801687RN) and imposed a $1,000 civil penalty for demonstrated incidents of dishonesty.  He resides in Marrero, La.
  • Mississippi—The Board accepted the Voluntary Surrender of Tammy L. Wells’s registered nurse license (202005692RN) for misrepresentation during the application process for licensure, and failing to answer questions truthfully.  She resides in Madison, Miss.
