The Board of Nursing meets 12 times a year and takes disciplinary actions during each meeting.  Listed below are the meetings held for the past year.  The status and any disciplinary documents for individual nurses or nursing assistants licensed in Oregon are available on the OSBN Verification System.  You can also view a List of Civil Penalties Imposed by the Board on Imposters and Facilities. 

​Oregon State Board of Nursing (OSBN) took final disciplinary action against the following nurses or nursing assistants during its March 19, 2025, Board Meeting.  The status of individual nurses or nursing assistants licensed in Oregon is available on the OSBN Verification System​​.  Or, you can download a file of all actions​​ taken during the meeting.

Douglas County
  • The Board reprimanded the nursing assistant certificate of Ethan R. Logo (202201425CNA) for falsifying data in a client record. He resides in Winchester Bay, Ore.
  • The Board imposed a $4,650 civil penalty against Anna M. Stratton's registered nurse license (201604930RN) for practicing nursing without a current license. She resides in Roseburg, Ore. (NCSBN ID # 23175526)

  • The Board accepted the Voluntary Surrender of Michael A. Lee's nursing assistant certificate (202007574CNA) for conviction of a crime that bears demonstrable relationship to the duties of a nursing assistant and abusing a person.  He resides in Medford, Ore. 
Linn County
  • The Board placed the family nurse practitioner license of Susan E. Cunningham (201608738NP-PP) on probation for 12 months for failing to properly assess and document client assessment when prescribing drugs and prescribing drugs in an unsafe or unlawful manner.  She resides in Albany, Ore.
