The Board of Nursing meets 12 times a year and takes disciplinary actions during each meeting.  Listed below are the meetings held for the past year.  The status and any disciplinary documents for individual nurses or nursing assistants licensed in Oregon are available on the OSBN Verification System.  Or, you can download a file of all actions taken during the meetings below.
 
​The Oregon State Board of Nursing (OSBN) took final disciplinary action against the following nurses or nursing assistants during its December 15, 2021, Board Meeting.  The status of individual nurses or nursing assistants licensed in Oregon is available on the OSBN Verification System​​.  Or, you can download a file of all actions​ taken during the meeting.

Clackamas County
  • The Board reprimanded the registered nurse license of Alexandria L. Engels-Smith (201607372RN) for violating a person's right to privacy and confidentiality and failing to conform to the essential standards of acceptable nursing practice.  She resides in Happy Valley, Ore.
  • The Board reprimanded the registered nurse license of Margaret A. Griffith (083043001RN) with conditions for conduct derogatory to the standards of nursing.  She resides in Lake Oswego, Ore.

Columbia County

  • ​The Board accepted the Voluntary Surrender of Tera L. Roberts's registered nurse and family nurse practitioner licenses (200941738RN/ 201150123NP) for prescribing drugs to people who were not her clients and for prescribing drugs for personal use.  She resides in Vernonia, Ore.

Curry County

  • ​The Board accepted the Voluntary Surrender of Deanna L. Chapman's practical nurse license (201804538LPN) for failing to answer questions truthfully on a licensure application.  She resides in Brookings, Ore.

Deschutes County

  • The Board imposed a $5,000 civil penalty against Alicia Michaelson for representing herself as an LPN in Oregon without a nursing license.  She resides in Bend, Ore.
  • The Board placed the registered nurse license of Jenny M. Hadley (201406567RN) on probation for 24 months for using intoxicants to the extent injurious to herself or others.  She resides in Bend, Ore.

Douglas County

  • The Board reprimanded the family nurse license of William F. Briggs (085072588N1) for failing to accurately document nursing interventions and failing to conform to the essential standards of acceptable nursing practice.  He resides in Sutherlin, Ore.
  • The Board accepted the Voluntary Surrender of Samantha J. Slobodiak's nursing assistant certificate (201902526CNA) for failing to answer questions truthfully on her renewal application.  She resides in Myrtle Creek, Ore.

Jefferson County

  • The Board suspended the registered nurse license of Jessica C. Fraley (20084226​2RN) for 90 days for violating the terms and conditions of a Board Order.  She resides in Klamath Falls, Ore.

  • The Board accepted the Voluntary Surrender of Marisia A. Daley's practical nurse license (201601737LPN) for violating the terms and conditions of a Board Order.  She resides in Eugene, Ore.
