• Page 9 - Initial and date next to the fee that represents your total organizational budget:
• Qualified entity - All non-special district entities (for example: city, county, tribe, school, public benefit, etc.)
• *No Fee Section - If your entity type is listed

If indicating an annual fee, include a one-page summary of your total organizational budget.
