Please order supplies at least 2 weeks before your stock is depleted. Seasonal rushes may cause delays in delivery.

Disease Testing Kits

The Oregon Department of Agriculture provides free sampling kits to Oregon accredited veterinarians for Highly Pathogenic Avian Influenza and New World Screwworm. These kits are intended to ensure that veternarians have necessary supplies and PPE on hand for sampling patients when Foreign Animal Diseases may be suspected. Suspected cases of any immediately reported disease must be reported to ODA prior to sample collection and laboratory submission.

Traceability Forms and Supplies

The following items are issued by the Oregon Department of Agriculture (ODA) to accredited veterinarians. To order supplies, complete the Veterinarian Supplies Order Form (be sure to include your name and USDA accreditation number – NAN) and return the form to ODA via fax (503-986-4734) or email oda.Animalhealth@oda.oregon.gov . Supplies may also be ordered through the OVIS online system.

Ordering Free RFID ear tags

Oregon accredited veterinarians may order tags directly from the Oregon Department of Agriculture (ODA) Animal Health Program using the veterinarian supplies order form.

A Premises Identification Number (PIN) must be included on the order form that matches the clinic’s address. PINs may be obtained by completing the premises registration application and returning it with the supplies order. Please visit our Premises Identification Registration page for more information.

RFID/EID tags can also be ordered directly from tag manufacturers. The USDA has a list of approved identification devices that can be viewed here on their Approved ID Device website.
