Process for Out-of-State Programs with Educational Experiences in Oregon

The Oregon State Board of Nursing requires all out-of-state pre-licensure nursing programs with anticipated educational experiences in Oregon to provide notification prior to the start of any learning activities, per OAR 851-021-0090(1). This includes clinical placements for practical nursing and registered nursing education programs.
  • Out-of-state education institutions that wish to establish a physical location in Oregon must follow the approval process described in OAR 851-021-0010.
  • Complete one form for each education track offered (i.e.: programs offering both PN and RN clinical experiences in Oregon; programs with different degree offerings leading to licensure)
  • Advanced practice programs and programs that do not lead to licensure (i.e. RN-to-BSN, non-advanced practice MSN or DNP programs) are not required to provide notification.

Education programs must complete the following form to provide the required notification; programs must resubmit information on an annual basis and may resubmit sooner if there are enrollment changes. Board staff will provide confirmation that your notification has been received. Education institutions must also ensure they have met the requirements set forth by the Oregon Higher Education Coordinating Commission or NC-SARA Authorization prior to enrolling Oregon students.

Individual students should not complete this form for themselves.
