This form is for currently enrolled nursing students. Do not use this form if you have completed an Oregon CNA 2 training program.
If you completed an Oregon-approved CNA2 training program, your instructor will notify the OSBN to add your CNA2 registration. If you have a question about your CNA2 status, please contact your instructor first.
You are eligible to apply for CNA 2 registration using the form below if:- You hold current CNA 1 certification in good standing.
- You are a current student in a RN or PN program.
- You have completed equivalent CNA 2 coursework.
Please Note: Not all nursing programs participate in the CNA2 program. Please speak with your Dean or Director to determine if your program participates. To register as a CNA2, nursing students must obtain a validation letter from their nursing program Dean or Director on school letterhead. The letter must state the student is currently enrolled in the school nursing program and has completed education and training equivalent to the CNA duties and standards as defined by Division 63 of the Oregon Nurse Practice Act.
