Entities those who qualify under ORS 802.175-802.191 to receive personal information, to apply for a DMV Record Inquiry Account. Established account holders may request records via DMV's Interactive Voice Response system, by fax, or by ordering online. Records ordered are returned by fax or mail. Account holders are billed for DMV services on a monthly basis. Click here for available DMV records and fees.
How to Apply

- Attach required documents (see application);
- Include the $70 fee; and

Automated Reporting Service
DMV's Automated Reporting Service is available to Record Inquiry Account holders with drivers under their employment. This service produces and mails the employer a Certified Court Print driving record when a conviction, accident, suspension, cancellation or revocation is posted to one of their employee's driving record.
Interactive Voice Response System
DMV's Interactive Voice Response System is an automated phone system available to Record Inquiry Account holders.
