Organ, Eye and Tissue Donation

Overview

This page provides information about the Donor Registry and becoming a donor.
 
All individuals 15 years of age and older can register as an organ, eye and tissue donor.
 
One donor can save up to 8 lives.
 
You can be a donor regardless of your health or physical condition, so do not rule yourself out. Criteria for donations change all the time due to medical research and advancements. The ability to be a donor is determined only at the time of death.​

How To Sign Up
To sign up on the Donor Registry:
  • Ask DMV to put a “D" code on your license, permit or identification card;
  • Go online at donatelifenw.org; or
  • Request a paper form by calling (800) 452-1369.
By signing up on the Donor Registry, you are confirming your commitment to be a donor and ensuring your decision is known and honored by your family and the organ, eye, and tissue recovery agencies.  It is extremely important you also inform your family of your wishes to be a donor.
