What is the Medical Monitoring Project?

Medical Monitoring Project logoThe Oregon Medical Monitoring Project (MMP) is a public health survey designed to learn more about the experiences and needs of people who are living with HIV. It is conducted by Oregon Health Authority (OHA) along with the Centers for Disease Control and Prevention (CDC). 

Information collected is used to increase awareness about different issues that impact people living with HIV. For example, MMP data has been used to educate health care providers on ways to improve care for persons living with HIV and guide treatment services in Oregon and across the nation. It has also been used to improve service delivery to people living with HIV in Oregon and identify unmet need, quality of care, and resilience factors.

  • DHS - Aging and Disability Resource Connection
    • Oregon's Aging and Disability Resource Connection (ADRC) helps people of all ages, incomes and disabilities learn about long-term support options in their communities.

  • HIV Day Center (Portland)
    • The HIV Day Center is a community center for low-income people living with HIV/AIDS.
    • Services include hot breakfasts and lunches five days a week, computers with internet, washer and dryer, shower and hygiene supplies.

Case Management

  • Find a case manager (Check website for most updated case manager contact information)
  • Case managers and care coordinators help people living with HIV access primary medical care and medications, identify and remove barriers to medical care, and ensure adherence to a prescribed treatment plan in a client-centered way that ensures timely and coordinated access.
  • Case manager and care coordinators can also see if you are eligible for other support services such as medical transportation and food/nutrition services. 

CAREAssist

  • CAREAssist is Oregon's AIDS Drug Assistance Program (ADAP). The program provides access to life-saving medications used to treat HIV and improve the overall health of people living with HIV who have limited resources or no health insurance coverage.
