Environmental investigation

  • Reimbursements for Home Investigations - Per OAR 410-151-0040, home environmental lead investigations may be reimbursable if the child is enrolled in the Oregon Health Plan. Read this article for more information or contact our program.

NOTE: Only certified risk assessors can perform environmental sampling (dust, paint chips, and soil) in pre-1978 housing to identify lead-based paint hazards.


Medical management for providers and county health department staff

Children

  • Medical Information Form - For children with elevated blood lead levels: County Health Departments contact the medical provider and have them complete this form. Copy of completed form needs to be sent to OHA along with all case management documentation.
  • Medical Evaluation and Recommendations - For children with elevated blood lead levels (EBLLs). This document is intended to provide evidence-based guidance for medical providers caring for children with confirmed EBLLs.
  • Management of Childhood Lead Exposure - this website provides recommendations from the Pediatric Environmental Health Specialty Units and the American Academy of Pediatrics.

Adults
