Amend a Payroll Report

​You can make adjustments to taxes reported electronically on your Form OQ or OA (Domestic). Go to Frances Online​ and log in to your account to adjust your reports.​ If you filed using paper forms, you'll need to complete the Form OQ/OA - AMENDED Report.

Important

If there is an adjustment to subject wages for Unemployment Insurance, in most cases, you’ll need to adjust your Form 132 Employee Detail Report. This can be done in Frances Online if you filed electronically. If you filed using paper forms, you must also submit a Form 132 - AMENDED Report. However, if your original Form 132 was filled out correctly, you will not need to do this.

You must also amend your OQ or OA if there is an adjustment to the total wages on the Form 132 Employee Detail Report. This can be done in Frances Online if you filed electronically. If you filed using paper forms, you must also submit an OQ/OA - AMENDED.

If you need to make changes to your semi-weekly or banking day deposits, complete the Schedule B Amendment form and submit it to the Oregon Department of Revenue. Address information is located on the bottom of the form.

If you need to make changes or corrections to social security numbers for payroll tax reporting purposes, complete the Notice of SSN Change-Correction form and submit it to the address located on the bottom of the form.
