Required Identity Documentation

For citizens, lawful permanent residents in the U.S. or citizens of a COFA Nation, you need each of these:

  • One proof of legal presence/identity. (Group A)
  • One proof of full legal name change, if your name has changed. (Group B)
  • One proof of your current residence address. This is the address where you physically live. (Group C).

For visitors in the U.S. for a temporary stay, you need each of these:

  • One or more official U.S. Immigration documents containing your full legal name and authorized length of stay date. (Group D)
  • One proof of your current residence address. This is the address where you physically live. (Group C)

For U.S. citizens, lawful permanent residents in the U.S. or citizens of a COFA Nation renewing or replacing, you may need each of these:

  • One proof of identity. (Group E)
  • One proof of full legal name change, if your name has changed. (Group B)
  • One proof of your current residence address. This is the address where you physically live. (Group C).

Important information

  • One document may satisfy more than one group.
  • New residents must surrender any out-of-state or out-of-country licenses/permits prior to getting an Oregon Driver License/Permit/ID Card.
  • All documents from Group A, B, D, and E must be originals and will be subject to verification. Temporary documents and photocopies of documents will not be accepted.
  • Documents accepted by DMV as proof of identity, legal presence, and Oregon residency may change without prior notice.
  • All U.S. citizens, lawful permanent residents of the U.S., and those authorized to work in the U.S. must provide a verifiable Social Security number, no exceptions.

Oregon DMV has the right to:

  • Ask for additional proof, or
  • Refuse any document.
