Effective May 17, 2021, the Board will require the completion of the Notary Public section on applications with a notary section.
Those who feel they cannot obtain the services of a notary may request a waiver, in writing, on a case-by-case basis by emailing boa.info@oregon.gov.

To change your address with the Board, you may submit a completed Address Change form by email to boa.info@oregon.gov
