Applications & payment may be submitted online using our secure online portal below
Applications and payment may also be mailed to the Board office at 200 Hawthorne Ave. SE Ste. D450 Salem, OR 97301
Applications submitted online or postmarked after December 31, 2025 will be assessed a late fee per OAR 801-010-0010(4).
Firm renewal applications are due December 31, 2025 and late renewals by January 31, 2026. Unfortunately, firms that do not submit a completed renewal application by January 31, 2026 will be terminated.
If your firm was previously terminated and you would like to resume business in Oregon, please contact the Board for the Firm Reinstatement application and submit to the Board according to the directions on the application.
