Public Records Request Form

The Department of Administrative Services (DAS) responds to all official requests for public records as soon as practicable and without unreasonable delay, according to timelines outlined in ORS 192.324 and ORS 192.329. DAS may charge fees for fulfilling public records requests according to the 
Statewide Policy on Public Records Requests Fees and Charges. For more information, view our FAQ.

Choose a method to request public records from DAS:

  1. Submit your request using the convenient submission form below. 
    • You will receive a confirming email that summarizes your request.
  2. Mail a written request to this address:

     Salem, OR 97301

To ensure the most expedient processing of your request, please submit your request using the online submission form. Other methods of submission will take longer to process.
