Public agencies not under DAS authority and Oregon Forward participants entering into a contract must submit this form, completed with authorized signatures, the statement of work and costing workbooks to Procurement Services for price determination.
Oregon Forward annual status reporting
All Oregon Forward participants must use this form to submit information to Procurement Services within 120 days of the end of their fiscal year.
