Insurance Certificate of Coverage

DAS Risk Management is changing the way certificates of insurance coverage are issued. This is being done to save the amount of time used by both agencies and DAS Risk Management to process and deliver these documents.

Much like how we currently provide Proof of Coverage for our state-owned vehicles, Risk Management is posting its all-purpose certificate of insurance coverage on our website for all to download or link to when proof of the state of Oregon's insurance coverage is requested. This is generally requested when the state has signed agreements and contracts as the service-provider or lessee. The certificate contains generic references back to the contract regarding the need for:
  • The insured name (state of Oregon in lieu of specific agencies, boards and commissions)
  • Description of certificate use
A new certificate will be posted on the website at least annually when the tort cap limits are changed on July 1st. Each certificate version is identifiable.
Fiscal Year 2627 Certificate (July 1, 2026 through June 30, 2027)

When agencies are renting equipment or venue space, rental companies will often request a certificate of insurance coverage that includes the following additional information:

  • Indemnification
  • Additional Insured Status
  • Lenders Loss Payable

The certificate below contains all the information of the generic certificate, plus additional verbiage related to these 3 topics in the description section. This certificate is intended to be used for equipment and venue rental only. If any of the additional information is requested of an agency in an agreement or contract not related to equipment or venue rental, please reach out to DAS Risk Management for further guidance. A new certificate will be posted on the website at least annually when the tort cap limits are changed on July 1st. Each certificate version is identifiable.

Fiscal Year 2627 Certificate (July 1, 2026 through June 30,2027)

The relevant certificate of insurance coverage should work. We no longer provide individualized certificates. Instead, we include in the section of the certificate labeled "Description of certificate use" language that should satisfy your need as long as you have a contract or agreement with your state agency and the certificate requestor listed.

If you have questions or have concerns raised by contract holders about our certificates, feel free to contact DAS Risk Management. You may also feel free to have your contract holders contact us directly with their concerns.

Agencies can request their own Workers' Compensation certificate by contacting SAIF Corporation directly. If you need information or have any questions, contact Kristie Marecek, SAIF's Oregon Team at  503-373-8107 or email her at krimar@saif.com
