Individuals interested in getting the COVID-19 vaccine and are 18 years old or older should complete this questionnaire to get your name on the list.

​Columbia County residents interested in receiving the COVID-19 vaccine can fill out the COVID-19 Vaccination Interest Form.  The information will be used to notify you when you are eligible to receive a COVID-19 vaccine.

Individuals must have an appointment in order to receive a vaccine.   The County Health Department will have vaccination clinics at Crook County Fairgrounds every Tuesday, starting February 9th. Please make an appointment at crookcounty.simplybook.me if you are in a current eligibility group.

​Curry Health Network is distributing COVID-19 vaccines to those in Phase 1a (and first two sub-groups of Phase 1b.  To join a waitlist to be notified of availability, click on the appropriate link below according to your age.
