How to Report Fraud

You can report suspected fraud for people receiving Oregon benefits. (Oregon Health Plan, food, cash or childcare).

You can report online, by FAX, or by mail. Please send only one report. Reporting online is the simplest option.

What happens after I report participant fraud?

  • A fraud referral coordinator will review your complaint.
  • We may contact you if we need more information.
  • The complaint will be forwarded to the ODHS Fraud Investigations Unit and assigned to an investigator, if appropriate.
  • To keep the information private, we aren’t able to report any information back to you.

Can I report people committing participant fraud in other states?

We can only review Oregon benefits. To report fraud in other states, contact that state’s agency or visit the United Council on Welfare Fraud (UCOWF) website. UCOWF is an international organization that fights fraud, waste and abuse in social service programs.​


What counts as participant fraud in Oregon?

Participant fraud against Oregon Department of Human Services or Oregon Health Authority programs includes getting benefits like cash, medical, food assistance or childcare a person isn’t eligible for.

For example, not reporting employment, where they get income, child support they receive, assets they own (homes, vehicles, boats, etc). It can include not reporting marriage, an absent parent in the home (including stepparents) or children living out of the home.

You can report suspected provider fraud, waste and abuse related to the Oregon Health Plan, which is Oregon’s Medicaid program.

What happens after I report Medicaid enrolled provider fraud, waste or abuse?​

  • ​Oregon Health Authority will investigate all reports of Medicaid enrolled provider fraud, waste and abuse.
  • If you give us your name and phone number, we may contact you if we need more information. If you want to stay anonymous, don’t give your name or number.
  • If we find that a provider committed fraud or abuse, we will refer the case to the Department of Justice or local district attorney. They will decide if they want to prosecute the provider.
  • If they decide not to prosecute, the case may be referred back to OHA for administrative action.

What kind of medical provider fraud, waste and abuse do you investigate?

We investigate all fraud, waste and abuse allegations made against OHA Medicaid enrolled providers and contractors.

Some examples include:

  • Billing for services that were not provided
  • Intentionally double ​billing for services
  • Billing for a higher level of service than what was provided
  • Billing for services provided by unlicensed or ineligible providers
  • Violations of the Anti-Kickback Statute

What if I’m not sure thi​s ho​tline is the right place to call?

Ask yourself the following questions:

  • Does the incident involve fraud, waste or abuse of state money or equipment?
  • Are Oregon Department of Human Services employees involved?

If you answer yes to both questions, please call the hotline.

If the suspected fraud involves people receiving Oregon benefits (Oregon Health Plan, food, cash, or childcare), use the Fraud Allegation Portal​.


Do I need to have evidence of the fraud or waste or just suspicion?

Non-specific reports of waste or fraud are difficult or impossible for us to investigate.​


What information should I provide?

  • Specific information, including names of people who might be involved or ​know about the matter
  • Dates, times and details of the incident
  • Any documentation you or others can provide
