renew is listed below.
You will renew your Medicaid agency
certification and
endorsements at the same time.
24-Residential and Host Homes also need to complete a separate license application for each site. See details below.
How to renew
Prepare your documents
- Gather all required documents before you start the online application. You can't save the application once you start.
- The online application takes about
20-30 minutes to complete.
- Before you start, know how many documents you have and their total size.
- A zip folder is the best option to upload your documents.
- Without a zip folder, you can only upload ten documents with a total size of 30MB.
Executive director's documents you will need
- Background check
- First aid and CPR certificates
- Training certifications for the two required mandatory reporting of abuse trainings. (All executive directors must complete training for both adults and children, regardless of which populations you serve.)

Use the Medicaid agency payment portal for all application fees.

Check the fee schedule to make sure you are paying the correct amount.
- All fees are nonrefundable and cannot be transferred.
- Payment is due when you submit your application.
- Save your payment receipt. You will need to upload it with your application.
Providers renew agency certification and endorsements at the same time. We contact you three to six months before your license expires to remind you to submit your renewal application. Review

Timeline and fees
Renewal cycle: Every
two years for both agency certification and endorsements
Renewal fee: $500 certification fee, plus separate endorsement fees

After you renew your certification and endorsement, you must submit a separate license renewal application for
each 24-Hour Residential site. We will then do an in-home inspection. If we find any violations, we will cite them and you must respond with a
Plan of Correction. Review

Request a change to your agency
Follow the
instructions below and use the
correct online form for the type of change you need to make.
How to request changes
Important
- Submit one change request per form.
- Gather your documents
before you start.
- You
can't save your progress once you start.
- Each form takes about
15-20 minutes to complete.
- All forms are available online and can be filled out electronically. Handwritten materials are
not accepted.
- Some changes require a nonrefundable fee.

- We may ask for more information or missing documents.

Watch your email. If we need additional information, you need to return documents by the deadline provided.

confirmation email when your changes have been completed.

Agency dissolution means the organization will
no longer operate in any capacity. All endorsements and licensed homes under the agency will end. The agency will no longer be authorized to operate.

Professional Behavior Support Services (for each Behavior Specialist)

- Current Oregon Intervention System (OIS) certification
- Redacted FBA with invoice
- Training certificates for the two required trainings on mandatory reporting of abuse
- Evidence of 12 hours of ongoing training (not required in first year)

To make any of the 24-Hour Residential license changes listed below:
Use the amendment form
Permanent age change
This means changing your license from supporting children to supporting adults, or vice versa. This change is permanent unless otherwise requested.
Required documents
- If converting from
adult to children's home, submit updated policies and procedures that meet all requirements listed in
OAR 411-325.
Temporary age change
This means changing your license to temporarily allow an adult to live in a children's home, or vice versa, for a certain period of time. The license changes back to the original age range after the time period has ended.
Required documents
- Safety assessments for each person currently living in the home, as well as for the individual who will be moving in.
- Work with the Case Management Entity (CME) to complete these assessments.
- We will contact the CME for their recommendation before granting approval.
- If a
child will be placed in an adult home, submit updated policies and procedures that meet all requirements listed in
OAR 411-325.
Capacity change
This means adding or reducing the maximum number of individuals you may support in the home.
If you are increasing capacity
Residential site closure
This means closing a 24-hour residential site location.
