If a form does not print as you see it on the screen, we will not be able to process it.
Some members experience difficulty printing the forms correctly when using a Macintosh (Apple) with the Safari browser. If you are experiencing this problem, downloading the free Internet browser Firefox
and using it to access and print PERS forms may resolve this issue.
If you are using a Mac (Apple) Safari browser, the PDF viewer does not support some PERS forms. You will need to make Adobe Acrobat or the Reader the default program for opening PDFs.
1. Download the latest version of Safari.
2. In Finder, select a PDF. Select File and Get Info.
2. Select either Adobe Acrobat or Reader from the Application menu.
3. Click Change All.
4. When asked if you want to change all similar documents, click Continue.
