Beneficiary forms

Need to name or update the beneficiaries for your PERS benefits?

The forms you need to submit depend on your benefit type:

• Tier One/Tier Two — Pension and Individual Account Program (IAP) account.
• OPSRP — Individual Account Program (IAP) only. Beneficiaries for your OPSRP pension are defined for you by Oregon law.

If you need to name additional beneficiaries, you will need to submit a completed Supplemental Insert to Name Additional Beneficiaries form with your beneficiary designation.

Submit beneficiary forms directly to PERS; turning them into your employer doesn't count.

If you still have questions, you can contact PERS at any time, but you may especially want to do this when major life events occur. Major events include marriage, divorce, or the death of a beneficiary.
