File a Public Records Complaint

Use this form to file a complaint under Oregon Code ORS 192.464.

  1. Include all relevant details such as dates, persons/offices contacted, and information requested.
  2. Attach a copy of your public records request, and any response received from the public body. If you are unable to do so at this time, consider pausing to do so before you submit this form. Otherwise, gather the relevant documents, including public records request(s), communication(s) with the public body, and response(s) from the public body.
  3. Include current contact information and respond promptly to communications from the Public Records Advocate. If we are unable reach you, that may delay or prevent the processing of your complaint.
  4. Check the box authorizing us to release relevant information to the necessary public bodies.
