RENEW Your SLP, SLPA, or Audiology License
The deadline to renew is December 31st, 2015. Please make sure your contact information is accurate so you receive your renewal notice in a timely manner.
Make sure your Professional Development will apply towards your license renewal by going to our PD page. Submitting a renewal with insufficient PD will cause your renewal to be incomplete and a delinquent fee will be charged. If your renewal is submitted late and/or you are selected for PD audit, this can result in multiple fees.
If you were just licensed in 2014-15, the amount of professional development due depends on your initial license date. If you had a Conditional License, it is the effective date you upgraded to a full SLP license.
For all others, the full professional development requirement is 30 hours for SLPs & Auds and 15 hours for SLPAs.
The renewal fee is $210 for SLPs & Auds and $65 for SLPAs.
Applying for an Initial SLP, SLPA, or Audiology License
Beginning June 1st, 2015 the Board will conduct a background check on all applicants for licensure. All applications will include the requirement to submit digital fingerprints through FieldPrint, Inc. The digitally captured fingerprints will be transmitted to the Oregon State Police and FBI. Instructions can be found on Supplement 2 of our applications.

page. You must submit the application with the application fee of $75. Including the license fee for your respective license can help speed the processing of the application. Transcripts, verification of your ASHA CCCs, and verifications of any license you have ever held, must come directly from the issuing institution. These documents can be sent prior to application and requesting them early can help speed the application process.
