Asbestos Notification and Application Forms

DEQ's asbestos program is now using a new electronic data management system called Your DEQ Online. 

Please start submitting notifications and fees electronically. All notification and application forms can be found through Your DEQ Online. For more information please visit Your DEQ Online Help page.

DEQ is very excited to roll out this new, electronic service to asbestos abatement contractors and training providers! If you have questions, please contact the asbestos specialist in your area. 

Notification forms

  • ASN-1 DEQ Project Notification
  • For abatement of friable asbestos-containing material. This form is now available in Your DEQ Online.  For more information please visit Your DEQ Online Help page. 
  • ASN-2 Annual Notification
  • For small-scale abatement of friable asbestos-containing material. This form is now available in Your DEQ Online.  For more information please visit Your DEQ Online Help page. 
  • ASN-3 Quarterly Reporting
  • Used with the annual small-Scale and annual nonfriable notifications. This form is now available in Your DEQ Online.  For more information please visit Your DEQ Online Help page. 
  • ASN-4 Asbestos Waste Shipment Report
  • ASN-5 Air Clearance Sample Results: Required by OAR 340-248-0270. This form is now available in Your DEQ Online.  For more information please visit Your DEQ Online Help page.
