Provider Listing

You must be approved for ERDC payment before you start providing care. Payment cannot be made for care provided before approval. 

Please note: Being approved to recieve ERDC payments is not the same as being a licensed child care provider

How to get listed and approved for ERDC

  1. Complete the Child Care Provider Listing Form (de7494).
    • You can print the form from this web page or get the form from your local ODHS office or from the family you will be providing care for.
    • Instructions are included in the form. Be sure to follow all instructions to avoid delays. Delays will affect when you can start to receive payments.
    • Need help filling out the form? Call the Direct Pay Unit at 800-699-9074.
  2. Send your completed form to the Direct Pay Unit by:
  3. Wait to hear from us that you are approved.
    • We will process your form and contact you if there are any issues.
    • After you are approved, we will notify you and send you a billing form. You need a billing form to be paid.
  4. Start providing child care.
    • You can start providing child care after you have been approved and you have a billing form.
    • DELC will not pay for child care you provided before you were approved.
