The Oregon Board of Optometry uses an online system for license applications and license renewals.
Online Application System
All new license applications are submitted through the Board’s online application system.
First-Time Users – Registration
If you have not previously used the online application system, you must register before applying.
- Select Register at the bottom of the application screen
- Registration requires payment of the applicable application fee
- Once registered, you may access and complete your application online

License Fees
Board fees are established in rule. A complete list of current fees is available in

on the Oregon Secretary of State website.

Several renewal-related forms are available as fillable PDF documents.

License Renewals – Important Information
- Applications to renew a current license are not available on this page
- License renewal notifications are sent to the email address of record approximately 45 days before the license renewal due date
- Please check your junk or spam folder for emails from the Board
