License Verifications & Lists
If you are an active or inactive status licensee and you need a license verification for another state or for credentialing, please make the request in the documents section in the Self-Service License Portal.
If you would like to order a list of active or inactive status licensees, use this Self-Service License Portal link and click "Document request". If you were previously licensed in Oregon, but are lapsed or retired, please use this link to request a license verification. NOTE: Active and inactive status OBO licensees DO NOT use this link.
Document requests and license verifications are processed as an online order and paid via credit card. Once the payment is processed, OBO will email the requested list within 2-3 business days.
Oregon does primary source verification of every applicant's optometry education.
