If you have questions about the license data shown in the system, please contact the Board.
Request a Formal Written License Verification
If you require a formal written license verification, submit a written request and the required $40 fee.
If you are an optometrist applying for licensure in another state and that state requires a specific form, please send the form to the Oregon Board of Optometry with your request.
Required E-Signature for Email Requests
Any licensee request for information or to update licensee data by email must include the Board’s required e-signature elements:
- Full legal name
- License number
- Last 4 digits of the licensee’s Social Security number
Order a Licensee List
To order a list of active or inactive status licensees, use the online license search system and select Document Request.
You may place the order online and pay by credit card. Once payment is processed, the Oregon Board of Optometry will email the requested list.

The Board performs primary source verification of the highest level of education for each licensee.
