Disciplinary Actions

Use the Optometrist Portal Search to see the legal documents associated with the Oregon Board of Optometry’s discipline cases for the stated individual. As discussed at the November 6, 2015 Board meeting, discipline cases from the last 15 years are included on the agency’s website. At the Board's September 9, 2016 meeting, the Board updated the website discipline policy that withdrawn orders will be considered on a case-by-case basis. Respondents can request an order to be withdrawn from the Board's website and the Board will consider each case and make an independent decision on each request.

Please see the disciplinary action list for any action taken over the last 15 years. Use this to find a licensee and then use the Optometrist Portal Search to review the document associated with the case. The Board meets quarterly. This list is updated after any meeting where there is disciplinary action taken.

If you’d like more information about a licensee or case, please complete a public records request form. There are details of the case and investigation that are confidential, by completing a public records request, the agency will release any public record documents associated with the request.
