Apply for Continuing Education Credit

Use the Oregon Board of Optometry’s electronic submittal system to request continuing education (CE) credit or clinical or surgical observation credit. Upload all required documents for the request to be reviewed, using meaningful file names and one file for each requested item.

How to Apply for Credit

Before submitting a request, review the requirements for the applicable credit type and gather all required documentation. Requests must be submitted within 12 months after the course presentation to be eligible for Board credit.

  • Use the Board’s electronic submittal system to submit the request and upload supporting documents.
  • Upload all requested materials so the Board can review the course or observation request.
  • Use meaningful file names and submit one file for each requested item.
  • If you are sponsoring a conference or multiple courses, submit a separate request for each individual course, even if the courses are held on the same date.
  • Use the same course title on the submittal form and on the CE certificate.
  • Do not upload zip files. If you use software other than Microsoft products or Adobe, save the requested information as a PDF document before uploading.

Submission Reminders

  • If you are sponsoring a conference or multiple courses, submit a separate request for each individual course, even if the courses occur on the same date.

Continuing Education Credit Requirements

Review the documentation requirements for each credit type before submitting materials through the Oregon Board of Optometry’s electronic submittal system.

Credit Type Requirements and Documentation
Course Credit (Live or Recorded)

When this applies

Structured continuing education courses, lectures, presentations, or other organized training activities.

Required documentation

  • Learning objectives (3–4 anticipated outcomes of the training)
  • PowerPoint slides or an itemized outline (approximately one page per hour of instruction)
  • CVs for presenters
  • Sample certificate of completion
  • Quiz questions for recorded or non-live courses
  • Checklist

Notes

  • Presenter CVs are not required for licensed health care professionals or university professors.
  • Recorded or non-live courses must include quiz questions.
  • Submit requests within 12 months after the course presentation.
Clinical Observation

When this applies

Observation of clinical activities or procedures in an approved educational or clinical setting.

Required documentation

  • Learning objectives (3–4 anticipated outcomes of the training)
  • Sample certificate of completion
  • Checklist

Notes

  • The observation site must meet Board approval requirements.
  • Submit requests within 12 months after the observation activity.
Surgical Observation

When this applies

Observation of surgical procedures for educational continuing education credit.

Required documentation

  • Brief description of the surgical procedure(s) to be observed
  • Learning objectives (3–4 anticipated outcomes of the training)
  • Sample certificate of completion
  • CV for each surgeon
  • Checklist

Notes

  • CVs are not required for licensed health care professionals or university professors.
  • Pre-approval may be required depending on the activity.
  • Submit requests within 12 months after the observation activity.
Non-COPE / External Course

When this applies

Educational courses that are not approved through COPE or AMA PRA Category 1 continuing education programs.

Required documentation

  • Full course packet and supporting materials
  • Learning objectives
  • Course outline or instructional materials
  • Sample certificate of completion
  • Checklist

Notes

  • Board review and approval are required before credit may be granted.
  • Additional documentation may be requested during the review process.
  • Submit requests within 12 months after the observation activity.
