Update Your License Record Online
Use the self-service portal to renew your license and maintain your license record.
Through the portal, you can:
- Renew your license
- Update your legal address of record
- Update your practice addresses
- Update your CPR certification
- Request license verifications
- Order license copies
- Print receipts
- Enter continuing education credits prior to renewal
License Verifications and Licensee Lists
If you are an active or inactive status licensee and need a license verification for another state or for credentialing, submit the request in the documents section of the self-service license portal.
If you would like to order a list of active or inactive status licensees, use the self-service license portal and select Document request.
If you are not a current licensee, you will need to register for an account to order records. If you were previously licensed in Oregon but are now lapsed or retired, use this same portal link to request a license verification. Active and inactive status Oregon Board of Optometry licensees should not use the non-licensee request path.
Processing and Delivery
Document requests and license verifications are processed as online orders and paid by credit card. Once payment is processed, the Oregon Board of Optometry will email the requested list within 2 to 3 business days.
